7th Grade Immunization Letter

Dear Parent/Guardian:

 

The purpose of this letter is to inform you of immunization requirements that will impact rising 7th-grade students. Students entering 7th grade are now required to have one dose of the Tdap (tetanus, diphtheria, pertussis) vaccine and one dose of meningococcal conjugate vaccine for individuals who have not already received those vaccines.

 

Please contact your health care provider or local health department if your child needs these vaccines. For additional information about the vaccines and diseases they protect against, visit the Center of Disease Control and Prevention’s website.

 

Please be prepared, on the first day of school, to provide proof of immunization (i.e., Certificate of Immunization [NC Immunization Report]).

 

Principals are required by law to prohibit any student from attending school until proof of immunization of required vaccinations has been supplied to the school office. Please be proactive and get your child’s vaccines completed.

A Note from William Lenoir’s school nurse:  

 

Please be sure that a copy of your child’s immunizations showing that the Tdap (Tdap/Pertussis) and Meningitis (Meningo) vaccines have been given is in our school office before the first day of school.  If we don’t have the actual immunization report, we don’t know that the vaccines were given and your child will be prohibited from attending school until proof is received.
